Job Summary:
Optimum Pediatric Services, LLC is a home healthcare agency that specializes in caring for children with medically fragile conditions. The Registered Nurse (RN) will travel to the patient’s home to provide care and support for the patient and family. This includes performing follow-up care per provider instructions and in accordance with scope of practice, assisting with patient and family education, and documenting clinical information into the medical record.
Essential Responsibilities:

Performs age and developmentally appropriate patient-family-centered care.

Utilizes clinical decision support tools and resources to recognize issues, prioritize care, and modify plan of care as needed.

Utilizes resources to plan and provide nursing care that is safe, effective, and financially responsible.

Participates in meeting educational needs of patients/families.

Participates in providing discharge instructions as delegated by licensed professional practitioners.

Demonstrates competencies that incorporate age-specific guidelines, including

N = Neonate (less than 30 days)

Primary Location:
Atlanta Metropolitan Area. Optimum Pediatric Services cares for patients in 45 counties across the state of Georgia, and the RN will be assigned patients in the counties nearest to them.
Schedule (Times May Vary):
Daily schedules may vary based on the client's needs. The daily schedule may include 8, 10, or 12-hour shifts. Daytime, evening, overnights. Weekdays, or weekends, or combinations.
